Simply explained

Endometriosis is a benign, usually chronic disease in which tissue that resembles the lining of the uterus (the endometrium) grows outside the uterine cavity.

How does endometriosis develop?

Normally, the lining of the uterus only lines the inside of the uterus. In endometriosis, this tissue is also found in other parts of the bodyThe most common causes are: on the ovaries (so-called endometriomas), on the peritoneum, in the uterine wall (adenomyosis) or, in rare cases, on the bowel, bladder or other organs in the abdominal cavity.

These Endometriosis centres react like the normal uterine lining to the hormones of the menstrual cycle. They build up during the cycle and bleed during menstruation. However, as the blood cannot drain away like during normal menstruation, inflammation, adhesions and pain occur.

Symptoms & disease progression of endometriosis

The symptoms of endometriosis can vary greatly. How severe the symptoms are does not necessarily depend on the extent of the disease. Some women with extensive endometriosis may have no problems at all, while others with minimal disease may suffer from severe symptoms.

Main symptoms:

Painful menstruation: Often increasing, cramp-like lower abdominal pain that begins before or with the onset of menstruation and can last longer than the usual one to two days

Chronic lower abdominal pain: Also occurring outside of menstruation, often dull and pulling

Pain during sexual intercourse: Especially with deep penetration, which may indicate infiltration of the posterior vaginal vault or the sigmoid rectum area

Infertility (infertility): About 30-50% of women affected by infertility have endometriosis

Other symptoms depending on localisation:

Intestinepainful defecation, cyclical bleeding in the stool, constipation, diarrhoea

Urinary tract/urinary bladder: Pain when urinating, blood in the urine, frequent urination

Rarer localisations: Pleura with cyclical pneumothorax, scar endometriosis after surgical interventions, e.g. caesarean section

Which women are prone to endometriosis?

Endometriosis can affect any woman of childbearing age. The disease usually begins at a young age, often in girls after their first menstrual period.

Risk factors and frequency

Around 7-15% of all women of childbearing age are affected by endometriosis. The disease can run in families, which indicates a genetic component. Women whose mothers or sisters have endometriosis have an increased risk.

Early first periods, short cycle lengths and childless women appear to be more frequently affected. Women with anatomical features that impede menstrual flow also have an increased risk.

What triggers endometriosis?

The exact causes of endometriosis have not yet been fully researched. Various theories are being discussed in endometriosis research:

Current research findings

One theory is that during menstruation, the lining of the uterus passes backwards through the fallopian tubes into the abdominal cavity and settles there. Another theory assumes that certain cells in the body can transform into endometrium-like tissue.

Hormonal factors, immune system disorders and genetic predisposition probably all play a role in the development of endometriosis. Endometriosis research is continuously working to better understand the causes and develop new treatment options.

Is endometriosis curable?

Endometriosis is a chronic disease that is currently not completely curable. However, very effective treatment options are available today that can significantly alleviate the symptoms and considerably improve quality of life.

Modern treatment approaches

The treatment of endometriosis is individualised and can include drug therapy, surgery or a combination of both. At our centre, we rely on evidence-based standards and take your personal wishes and life situation into account.

We only use minimally invasive endoscopic techniques for operations. Our principle of „as much as necessary - as little as possible“ means that we perform operations as gently as possible and endeavour to preserve organs. Most patients can leave the hospital after 1-3 days and quickly return to their everyday lives.

When is surgery necessary for endometriosis?

The decision in favour of surgical treatment of endometriosis is always individual and depends on your personal situation. Surgery should be considered for

Chronic pain:

Severe menstrual pain that does not respond adequately to painkillers

Pain during sexual intercourse

Chronic lower abdominal pain outside of menstruation

Pain during bowel movements or urination

Unfulfilled desire to have children:

Suspicion of endometriosis-related infertility

Adhesions in the area of the fallopian tubes and ovaries

Endometriosis cysts (so-called chocolate cysts) on the ovaries

Organ infestation:

Deep infiltrating endometriosis affecting the bowel, bladder or ureter

Impending organ damage

Bleeding disorders due to adenomyosis

After a detailed diagnosis, which includes a gynaecological examination, ultrasound and, if necessary, MRI, we will work with you to create an individual treatment plan. In doing so, we take into account your life situation, your desire to have children and your personal needs.

Surgical endometriosis treatment at the Geisenhofer Clinic

Our experienced team of highly certified surgeons (MIC III) has decades of expertise in the treatment of even the most complex endometriosis cases. As one of the largest endometriosis centres with the most complex cases in the DACH region (benchmarking annual report of the Endometriosis Research Foundation), we offer you the entire spectrum of modern surgical techniques.

Endometriosis resection in the desire to have children

For women who wish to have children, organ-preserving and fertility-sparing surgery is the top priority. During laparoscopic endometriosis resection, we precisely remove all visible endometriosis lesions while sparing healthy tissue and the reproductive organs as much as possible.

Our approach:

Careful inspection of the entire abdominal cavity

Removal of all endometriosis centres using special techniques

Restoration of normal anatomy

Checking the patency of the fallopian tubes

Use of adhesion barriers for prophylaxis

The operation is minimally invasive and involves small abdominal incisions (five to ten millimetres in size). State-of-the-art HD camera systems and special instruments enable us to work precisely and gently on the tissue.

Loosening adhesions

Endometriosis-related adhesions can lead to pain, organ dysfunction and infertility. Laparoscopic adhesiolysis (removal of adhesions) is an important part of endometriosis surgery.

Technique and procedure:

Careful separation of the adhesions with special instruments

Restoration of free movement of the organs

Special care in the area of the fallopian tubes and ovaries

Application of modern anti-adhesion barriers

Our gentle surgical technique minimises the risk of new adhesions and optimises your chances of pregnancy.

Removal of deep infiltrating endometriosis

Deep infiltrating endometriosis poses a particular challenge as it grows in depth and can affect other organs such as the bowel, bladder or ureter. The highest level of expertise is required here.

Even with extensive infestation, we can perform minimally invasive surgery in most cases. The complete removal of deep infiltrating endometriosis often leads to a significant reduction in pain and an improvement in quality of life.

Treatment of endometriosis cysts

Endometriosis cysts (endometriomas or „chocolate cysts“) on the ovaries can impair the egg reserve and should be operated on particularly gently if you wish to have children.

Our operational strategy:

Careful exfoliation of the cyst while protecting the ovarian tissue

Haemostasis by means of bipolar coagulation

Sclerotherapy using highly concentrated alcohol to maintain a maximum ovarian reserve

If you want to have children: combination with fertility diagnostics

After the operation, we will monitor your ovarian reserve and advise you on the best time for pregnancy.

Certified endometriosis centre

The MIS Centre (Centre for Minimally Invasive Surgery) at the Dr Geisenhofer Women's Clinic in Munich has been a certified endometriosis centre since 1 July 2015.
