Causes of adhesions in the abdomen

Adhesions are connective tissue adhesions between organs or tissues that should normally be able to move freely. They develop as a reaction of the body to various stimuli:

Common causes:

  • Previous operations: Any abdominal surgery can lead to adhesions, especially after open surgery
  • Inflammations: Appendicitis, ovarian inflammation or other infections in the abdominal cavity
  • Endometriosis: One of the most common causes of adhesions in women of childbearing age
  • Injuries: Trauma or injuries to the abdominal cavity
  • Radiotherapy: After radiotherapy in the pelvic area

Adhesions can affect various structures:

  • Fallopian tubes and ovaries (most common cause of mechanical sterility)
  • Uterus and pelvic wall

Surgical diagnostics for unfulfilled desire to have children

At about 20-40% all women with an unfulfilled desire to have children have adhesions or other anatomical changes. Laparoscopic diagnosis is the gold standard for clarification and simultaneous treatment.

Indications for a diagnostic laparoscopy:

  • Unfulfilled desire to have children for more than twelve months (for women over 35: six months)
  • Repeated miscarriages
  • Suspected tubal occlusion in diagnostics
  • History of risk factors for adhesions
  • Unclear lower abdominal pain

Procedure for surgical diagnostics:

  1. Preparation: Detailed medical history and gynaecological examination
  2. Operation: Minimally invasive procedure under general anaesthetic
  3. Inspection: Systematic examination of the entire pelvis
  4. Chromopertubation: checking the patency of the fallopian tubes with a blue colour solution
  5. Documentation: Photo and video documentation of all findings
  6. Therapy: Immediate treatment of pathological findings
